Our Process
Referral and Assessment:
The admission process begins with a referral from a hospital, healthcare provider, or the individual and their family. A pre-admission assessment is conducted to evaluate the resident's medical condition, functional abilities, and specific care needs.
Verification & Financial Info:
The facility will collect and verify insurance information to determine coverage for skilled nursing care. Any co-payment or out-of-pocket expenses will be reviewed
Care Plan Development:
Our interdisciplinary team will construct a care plan based on the resident's needs and include details about medical care, rehabilitation, medications, and activities of daily living.
Coordination of
Care Services:
The facility collaborates with other healthcare providers and professionals involved in the resident's care, such as physicians, therapists, and social workers, to ensure a seamless transition.
Ongoing Monitoring & Adjustment:
Monitoring of the resident's health and adjustment to the facility is conducted, and the care plan is periodically reviewed and adjusted to meet changing needs and goals.
Our Admissions and Social Services Departments will guide you through the Admissions process, answer questions, and address your concerns. They will also walk you through the confusing and, at times, daunting insurance reimbursement process.
At Steere House, each resident has the same care team daily to ease you through the transition. Medical, social, and rehabilitation specialists conduct a clinical evaluation to establish an individualized care plan centered around your specific needs.

Most of the staff have been with us a long time and all have an uncommon commitment to their work. They treat every resident as they would family.
