At Steere House, we offer experienced rehabilitation for patients who require care following a hospital stay. From reliable on-site case management to expert nursing, to innovative rehabilitation techniques, our goal is to maximize functional abilities and have the patient achieve their highest level of independence for a smooth transition before returning home or to the most appropriate community setting. We are staffed by highly skilled therapists and licensed clinicians who encourage continuing recovery. Our Carelink Rehab Team is experienced in physical, speech, and occupational therapies focused on recovery and restoration of independent daily living.

Meeting Individual Needs
Steere House will admit those patients/residents whose individual needs can be met by the staff and services available. A prospective patient/resident’s needs shall be thoroughly evaluated, and the least restrictive alternative settings considered before acceptance for admission. The evaluation shall be a joint responsibility of the referring agency or institution, the admitting physician, and the nursing home as the receiving facility.
Any restrictions, priorities, or specific admission criteria shall be applied equally to all potential admissions regardless of the source of referral, race, creed, ethnic origin, gender, or gender orientation, in compliance with State and Federal anti-discrimination statutes.
Referrals of residents for Steere House shall be accepted from hospitals, other nursing homes, self-referrals, relatives of residents, guardians, officials of State and County agencies, community agencies, religious agencies, and physicians.
